Parents and guardians may submit concerns related to sex-based harassment, sexual harassment, retaliation, discrimination, or failures to provide equitable educational access under Title IX. Families are encouraged to keep written records of all communications and requests.

I am writing to formally request investigation into concerns involving my child, [Student Name], under the district’s Title IX policies and procedures.
Based on the information currently available to our family, we believe the reported conduct may fall within the scope of Title IX protections and warrants a prompt, equitable, and adequately documented response.
At this time, we are requesting:
Confirmation that this matter has been received
The name and title of the assigned Title IX Coordinator or investigator
Information regarding the district’s formal investigation and resolution process
Written information regarding available supportive measures and accommodations
Clarification regarding anticipated communication timelines and procedural next steps
Written findings, determinations, and outcome documentation at the conclusion of the process, to the extent permitted by law and policy
For documentation purposes, we request that communications and determinations related to this matter be provided in writing whenever possible.
